Instructions for completion

1. List the people you believe you may have harmed.

2. Mark the type of harm caused: physical, mental, emotional, or spiritual.

3. Write the defects involved. This may include selfishness, dishonesty, fear, inconsideration, resentment, pride, avoidance, manipulation, or any other defect connected to the harm.

4. Mark whether you are currently willing to make amends. This does not mean making the amend today. It means becoming willing.

5. Review this list with a sponsor or trusted recovery guide before taking direct action.
